Advance Care Planning
We’re here to help you understand your options, ask the right questions, and have the right conversations. Through the Advance Care Planning process, you can assign a patient advocate and record an Advance Directive. If you become unable to advocate for yourself, your patient advocate and Advance Directive will enable us to follow your wishes.
Your Advance Care Planning Team
Our trained staff – which can include Social Workers and/or a Behavioral Psychologist – will guide you through the planning process and answer any questions you may have moving forward. They can help you adjust to life with cancer and provide additional support about your financial and practical concerns.
